Diabetes and Heart Disease Prevention Program

About the program

Did you know that diabetes and heart disease are among the most common causes of death in the United States? MCHC integrated diabetes and heart disease prevention education into its home-visiting programs to help individuals and families prevent risk factors and take control of their health.

Home-visiting staff not only provide one-to-one education during home visits, but also host in-person and virtual group sessions throughout the year that are free and open to the community. The program covers:

  • Type 1, 2, and gestational diabetes, heart disease, and how they develop.
  • Managing and understanding health parameters like blood pressure and blood sugar, questions to ask the doctor.
  • Practical ways to incorporate and keep track of exercise in everyday life and why it’s important.
  • Nutrition education like serving sizes, reading nutrition labels, the role of proteins, carbs, and other macronutrients.
  • Shopping healthy on a budget and cooking demonstrations with common ingredients.
